Getting your own Medicare card
You can have your own Medicare card if you’re aged 15 or older and enrolled in Medicare.
You can get your own Medicare card online or apply for one using a form.
We’ll send your card to the address you’ve given us. It’ll arrive in about 3 to 4 weeks. You can use a digital copy of your Medicare card in the meantime.

Keep in mind, you can only be on 2 Medicare cards at the same time.
How to get a Medicare card online

You can get your own card online if you meet all of these requirements:
- you’re living in Australia and you’re an Australian citizen, New Zealand citizen or you have a permanent resident visa
- you’re 15 or older
- you’re only on one Medicare card
- your current Medicare card has other people on it
- you’re not the contact person of your current Medicare card
- you want your own Medicare card with just your name on it.
If you don’t meet all of these requirements, you need to complete and send us a form.
How to get a Medicare card using a form
To make sure it’s you, we’ll ask for some identification. The types of identification you can provide are on the form.
You don’t need to get your document copies certified.
Send your form and documents to us. You can post them to the address on the form or email them to Medicare Enrolment Services.
You can get a new Medicare card if it expires or is lost, stolen or damaged. You can also get an extra card.
