Supporting documents

When you apply to be a Centrepay business, you need to provide supporting documents with your application form.

We’ll need supporting documents to help us assess the suitability of your business for Centrepay, when you submit your completed application.

It’s important you read the application form carefully and answer all the relevant questions.

When lodging, the combined file size of your application and supporting documents must not exceed 5mb. You can lodge up to 10 files.

What type of documents we ask for

To support your Centrepay business application, you’ll need to verify your identification before you can submit your application.

You’ll also need to provide us with supporting documents based on your business type and circumstances. This could include any of the following:

  • business structure
  • type of goods or services you provide
  • how you intend to use Centrepay.

All Authorised Officers, Contact Officers and persons signing the application form will be required to verify their identity with our online identity verification system.

You’ll need to register with the Provider Digital Access system and provide us with your individual Registration Authority number.

Complaints policy

You’ll need to adapt or create a Centrepay complaints policy for your business. Your policy must meet our minimum requirements.

If you don’t have a complaints policy, you can create one using our complaints policy template.

Lay-by policy

Copy of your business lay-by policy.

Incorrect payments policy

You’ll need to create a written policy describing how your business identifies, prevents, manages and rectifies incorrect payments. Your policy must meet our minimum requirements as outlined in the Centrepay policy, terms and guides.

Corporate structure

If your business has more than 5 related bodies corporate, you need to provide a structured diagram showing the corporate relationships between the related bodies corporate and the applicant.

Authorisations to operate the business or to provide the goods or services

Authorisations for specific licenses or permits for your business to legally supply the goods or services.

This could also be evidence of why the business is not required to hold any authorisations.

Leasing arrangements for accommodation providers

Head lease agreement or primary property lease agreements could include the following:

  • approval for subletting
  • special Disability Accommodation agreements for participants
  • variation of a tenancy or rooming agreement entered with the business.

Loan agreements

Copy of current loan agreements offered by your business

Evidence of revenue for remote & very remote community stores

If you are a remote store and sell meat or meat products, you need to submit evidence of revenue collected from those products in the last financial year.

If your business is new, you need to provide an estimate based on projected sales.

If you primarily sell meat products, you won’t be approved as a Centrepay business.

If we need more documents

To help us assess your application we may need more supporting documents, depending on the information you’ve told us.

We’ll contact you for additional documents by both:

  • calling you
  • sending a letter to the email address in your application.

Depending on our request, you will have up to 10 business days to respond.

It’s important to complete your application correctly. You might need to lodge a new application if:

  • the application form hasn’t been signed correctly
  • any amendments are not initialled
  • the application is partially or mostly incomplete
  • there are missed questions in the form
  • documents are missing and not provided to us in time.
