When to use this form

Only use this form if your proposed nominee is outside Australia.

If you’re appointing a payment nominee

If you would like to appoint a payment nominee and their bank account is held outside Australia, your payment nominee must also complete an international bank account form.

When not to use this form

Do not use this form if your proposed nominee is in Australia. Instead, download and complete the Authorising a person or organisation to enquire or act on your behalf form.
