Complete immunisation history statement
This immunisation history statement shows all immunisations reported to the Australian Immunisation Register (AIR) on or after 1 January 1996.
You can use your immunisation history statement to:
- keep track of the immunisations you’ve had
- see when any immunisations are due or overdue
- see if you have an immunisation medical contraindication or natural immunity recorded
- see if your child is on an active catch up schedule
- prove your immunisations are up to date for school, child care, employment or travel.
COVID-19 and influenza (flu) immunisation history statement
This immunisation history statement only shows your COVID-19 and influenza vaccination information recorded on the AIR.
You can also see if you have an immunisation medical contraindication recorded for any COVID-19 or flu vaccines.
If you have a child under 14 years old, you can also get their immunisation history statements. You can also check if your child meets the immunisation requirements for:
- Child Care Subsidy
- the full rate of Family Tax Benefit Part A
- Family Tax Benefit Part A supplement.
Your recent vaccinations may take a few days to appear on your immunisation history statement. If an immunisation is missing from your statement, ask your vaccination provider to update the AIR.
