Use this form to give us information about your partner if you are claiming or receiving a payment or concession card.
Employment income reporting
You must tell us the gross employment income you and your partner get so we can pay you the right amount.
Relationship details - Separated under one roof form (SS293)
Use this form if you’re separated but live under the same roof as your ex-partner. This is so we can assess whether to pay you as a single person or as a member of a couple.
