Return the completed form and supporting documents we ask you for to a service centre. If your organisation is approved, we will store your information on a Centrelink data base. We'll contact you to confirm the decision.
If you've not heard from us and want to check the progress of your request, visit the service centre where the form was submitted.
If you have a disability or impairment and use assistive technology, you may not be able to access our forms. If you can’t, please use self service, request someone to deal with us on your behalf, or contact us. We can help you access, complete and submit them.
