You can submit your form and supporting documents to us by mail or email to Medicare Enrolment Services. To help us process your request please include Medicare enrolment in the subject line. You don’t need to get your supporting document certified.
You don’t need to complete this form if you enrol your baby in Medicare when you claim:
If you don’t claim any of these payments, you can enrol your baby in Medicare using your Medicare online account through myGov. This is quicker and easier than completing this form. You can give us all of your baby’s details and proof of birth online. Read more about how to enrol your baby in Medicare.
To fill in this form digitally you will need a computer and Adobe Acrobat Reader, or a similar program. You can download Adobe Acrobat Reader for free. If you can’t complete the form digitally, you can print it, complete it by hand and return it to us following the instructions on the form.
