You can securely upload an image of your Centrelink form or other documents online.

Medical certificates

Follow these steps to upload a medical certificate using your Centrelink online account.

All other documents

To upload a document, follow these steps:

  1. save the document you want to upload on your mobile device or computer
  2. sign in to myGov and select Centrelink
  3. select Upload documents from your homepage
  4. select either Centrelink form and enter the form title or code or Other documents, then the category that best fits your document
  5. when your documents are ready, select Submit documents.

You can drag and drop your files into the Drop files here box or choose Select your file.

To check the documents you’ve uploaded, select MENU on your homepage. Then select Documents and Appointments, followed by Documents and View upload history.

Medical certificates

Follow these steps to upload a medical certificate using the Express Plus Centrelink mobile app.

All other documents

To upload a document, follow these steps:

  1. select Upload documents from your home screen, then Upload document
  2. select either Centrelink form followed by Scan barcode or Other document followed by:
    • Take a photo, Upload a document or Add a photo from library if you use an Apple device
    • Upload a photo or Upload a document if you use an Android device.
  3. when your documents or photos are ready, select Submit.

To check the documents you’ve uploaded, select Upload documents from your home screen.

What documents you can upload

The total size of all files for a document upload must be under 5MB. Each file must also be in one of the following formats:

  • pdf
  • png
  • jpeg
  • tiff.

You can include up to 10 files for each upload.

Make sure you remove the password protection on your document before you upload it.

Here are examples of the types of documents you can upload from your mobile device or computer.

Non-Centrelink medical certificates.

Medical documents such as:

  • a letter from a doctor or hospital
  • medical reports.
Accommodation

Rent certificates.

Proof of asset ownership documents.

Rates notices.

Rental contracts.

Child related documents

Newborn child claim proof of birth forms.

Birth certificates.

Adoption or surrogacy agreements.

Child care receipts.

Change of care documents.

Income Stream Review Any documentation used to support an Income Stream Review.
Immunisation evidence

Immunisation History form.

Medical Exemption form.

A Child Health record book or overseas documents are not acceptable proof of immunisation.

Foreign country documents

Certificate of income from overseas.

Certificate of residence status.

Letters from foreign governments.

Enquiry letter

Letter or fax.

Letter or fax to Centrelink from a third party.

Legal

Family Court documents.

Letters to confirm care of children.

Payslip Payslips.
Proof of identity Documents to confirm you are who you say you are.
