How to add or remove someone from your card
You can update your card when your circumstances change.
Adding someone to your card
If you’re a parent, you can add your child to your card. If you want to add someone aged 15 years or older, they’ll need to sign a form. You choose to either copy or transfer them to a new or existing card.
When you copy someone to your card, their name will be on both your card and their original card. This means both cards remain valid.
When you transfer someone to your card, they’ll appear on your card. This means their original card is no longer valid.
You can only be on 2 Medicare cards at the same time. For example, a child could appear on both their parents’ cards.
What you need to do
Complete an Application to copy or transfer from one Medicare card to another form. You need to list all the people you want to copy or transfer. Anyone aged 15 years old or older must sign the form.
The applicant listed as person 1 on the form needs to visit a service centre. They must bring with them:
- the completed form, signed by all those listed who are 15 years of age or older
- their own identification.
The types of identification you can bring with you are on the form.
Only post your application to us if:
- you live in an area remote from a service centre
- there’s a medical reason you can’t attend.
Use the address on the form to post your application and any certified copies of documents to us. You’ll need to tell us in writing why you can’t attend a service centre.
How to add your baby to your card
A newborn baby is a child aged up to 52 weeks or up to the day of their first birthday. You should enrol your baby in Medicare as soon as possible. You can enrol your newborn baby by using the Newborn Child Declaration. You’ll get the declaration from the hospital. Find out how to enrol your baby.
We’ll add your baby to your Medicare card once they’re enrolled. We’ll send you a new Medicare card with your baby’s name on it. You can get a digital version of your updated card immediately in the Express Plus Medicare app. If you want to add them to another card you’ll need to complete a form. Removing someone from your card
You can’t remove someone from your card. However you can request a new card. For example, a family member dies or you go through divorce, you’ll need to request a new card.
If your family circumstances change, such as through divorce, you can’t remove someone from your card. You can request a new card for yourself and your children.
What you need to do
Complete an Application to copy or transfer from one Medicare card to another form. Anyone aged 15 years old or older must sign the form.
You’ll get a new Medicare number straight away and we’ll send your card in 3 to 4 weeks. We’ll send it to the address you give us.
You can use a digital copy of your Medicare card until your new card arrives in the mail. To use a digital version of your Medicare card, you need to:
- Sign in to the Express Plus Medicare app.
- Select the Card tab on the homescreen.
