Add or remove someone from your Medicare card
You can update your card when your circumstances change.
Adding someone to your card
If you’re a parent, you can add your child to your card. If you want to add someone 15 years or older, they’ll need to sign a form. You choose to either copy or transfer them to a new or existing card.
When you copy someone to your card, their name will be on both your card and their original card. This means both cards remain valid.
When you transfer someone to your card, they’ll appear on your card. This means their original card is no longer valid.
You can only be on 2 Medicare cards at the same time. For example, a child could appear on both their parents’ cards.
What you need to do
Complete an Application to copy or transfer from one Medicare card to another form. You need to list all the people you want to copy or transfer. Anyone 15 years old or older must sign the form.
The applicant listed as person 1 on the form needs to mail or email Medicare Enrolment Services both of these:
- the completed form, signed by all those listed who are 15 years of age or older
- their own identification.
The types of identification you can provide are on the form.
How to add your baby to your card
We’ll add your baby to your Medicare card once they’re enrolled. You should enrol your baby in Medicare as soon as possible. You can enrol your newborn baby by using the Newborn Child Declaration. You’ll get the declaration from the hospital.
When you enrol your baby, we’ll send you a new Medicare card with your baby’s name on it. You can get a digital version of your updated card in the Express Plus Medicare app.
If you want to add your baby to another card you’ll need to complete a form. Complete the Application to copy or transfer from one Medicare card to another form.
Removing someone from your card
You can’t remove someone from your card. However you can request a new card. You may want to do this if a family member dies or you get a divorce.
If your family circumstances change, such as through divorce, you can request a new card for yourself and your children.
What you need to do
Complete an Application to copy or transfer from one Medicare card to another form. Anyone 15 years old or older must sign the form.
You’ll get a new Medicare number and we’ll send your card in 3 to 4 weeks. We’ll send it to the address you give us.
You can use a digital copy of your Medicare card until your new card arrives in the mail. To use a digital version of your Medicare card, you need to:
- Sign in to the Express Plus Medicare app.
- Select the Card tab on the homescreen.
