Changes to your circumstances

You need to tell us about changes in your situation. These may affect payments you get under the Extraordinary Assistance Fund or the Health Care Assistance Fund.

Changes may include:

  • support you get from the National Disability Insurance Scheme
  • support from other state, territory or Australian Government funded programs
  • your contact details
  • your address
  • your bank details
  • your accredited or registered health care practitioner or business details.
