For patient claims ask your patient to download and complete the Medicare Claim form. Your patient can use this form to claim benefits by mail or in person at a service centre.
Private health insurers or approved billing agents can use the Simplified Billing manual claims batch header form to submit a manual Simplified Billing claim.
Submit a manual DVA claim
You can submit a manual DVA claim by mail.
Use the Application for Department of Veterans’ Affairs claim adjustment form when applying for a Department of Veterans’ Affairs (DVA) claim adjustment for assigned DVA benefits where the original date of service is less than 2 years old.
It’s important you send all the required documents with your manual claim otherwise there’ll be delays in payment.
