Medicare Two-way claim form (MS001)

Use this form together with a Medicare claim form to submit your in-hospital and ancillary claims with Medicare and your private health fund.

Download and complete the Medicare Two-way claim form.

This form can also be used to claim gap benefits from your private health fund for in-hospital and ancillary services.

If your myGov account is linked to Medicare, you can submit your claim online.

To do this:

  1. Complete a Medicare Two-way claim form.
  2. Sign in to myGov.
  3. Attach an image of your original accounts and receipts if paid.
  4. Attach an image of the completed Medicare Two-way form.
  5. Submit your claim.
