You must give us one of the following:
- a signed Medical Confirmation form from your medical practitioner confirming you need the medical equipment
- evidence that the person with medical needs, or their carer, currently gets help for the same equipment through a state or territory government scheme. This doesn’t apply to claims for medically required heating or cooling.
You must tell us within 14 days of any change in your circumstances. Learn about the types of changes you need to tell us about.
We may also ask you for one or more of the following:
- evidence that you or the person you care for pays the energy costs to run the equipment, such as an electricity or gas bill
- a signed Equipment User Declaration form if the payment is to be made to your carer
- proof that you qualify for help under a state or territory government scheme
- evidence that the Department of Veterans’ Affairs supplied the medical equipment through the Rehabilitation Appliances Program.
