From 30 November 2026, we’ll stop providing printed aged care calculation of your cost of care forms through our print ordering system.
Your care recipients, or their correspondence nominee, need to apply online for the aged care calculation of your cost of care.
They can apply online through myGov by accessing Centrelink, starting a claim, and selecting the aged care calculation of your cost of care.
You can download and print the form from our website if needed. You should use the current version to avoid delays for care recipients.
