To complete the questionnaire, fill in the attached form and return it to us within 28 days of the date on your letter. You can complete the form by:
- calling the AIC line on 132 318
- uploading it to your Centrelink online account through myGov or the Express Plus Centrelink app
- mailing it to the address on the form.
