You don’t need to use this form if you’re accessing Support at Home services.
You can opt in if any of the following apply:
- you were approved for, or receiving home care on 12 September 2024
- you were in residential care on 31 October 2025 and haven’t spent more than 28 days outside of care, other than approved leave (although exceptions may apply in some circumstances)
- you move to a new service, or you’re staying in the same service and wanting to opt in on or after 1 November 2025.
