From 1 November 2025, Support at Home replaces the Home Care Packages Program. Dementia and cognition supplement will only be available to eligible grandparented home care recipients with a diagnosis prior to 1 November 2025.
You can use the Aged Care Provider Portal (ACPP) to do business with us.
You’ll need an individual Provider Digital Access (PRODA) account to access the ACPP. If you don’t have an account, you can register for an individual PRODA online.
Once registered, you can then register for home care supplements and subsidies using your ACPP.
The supplement is available only to eligible grandparented home care recipients who continue under Support at Home and had a diagnosis before 1 November 2025. Note: Eligible grandparented home care recipients who continue under Support at Home will continue to receive the supplement. If they are reassessed and accept a Support at Home classification, the supplement will cease.
To register for the dementia and cognition supplement, select the care recipient profile and:
- Go to the Supplements menus and select Dementia and cognition then select Register dementia supplement.
- Select Confirm to continue.
- Enter the Date of diagnosis and Diagnosis date of receipt. Then select Submit.
- Select the Held status hyperlink, and then select Go to dementia and cognition documents to upload the completed Aged care application for the dementia and cognition supplement in home care form (AC014).
- Upload any other required documents.

To register for the Enteral feeding supplement, you’ll need to be on a Care recipient record.
- Go to the Supplements menus and select Enteral feeding summary then select Register Enteral feeding.
- Select Confirm to continue.
- Enter the Start date on the Register enteral feeding screen.
- Select Yes or No for Bolus and select the relevant Enteral feeding type. If the higher rate Enteral feeding type option is selected, the screen will display Net Value (daily) field. Enter the Net Value (daily) amount in dollars and cents. If the start date is from 1 November 2025, the higher rate Enternal feeding type will no longer be applicable.
- A success message will be displayed to confirm successful registration and that the event is being processed.
- Select Go to enteral feeding documents to upload the form and supporting documents.
- Select the Document type and then select Go to enteral feeding supplement documents to upload the completed Application for the oxygen and/or enteral feeding supplement form (AC011).
- Upload any other required documents.

To apply, your care recipient must complete the Aged Care Claim for financial hardship assistance form (SA462).
We assess each application and consider things that are unique to each care recipient. Care recipients will also need to give us documents showing:
- some of their aged care costs
- their essential expenses
- any assets they can’t sell or borrow against, if applicable.
We assess financial hardship assistance claims within 28 days of getting all the information. We’ll send a letter to you and your care recipient to let you know if we have granted the claim. We’ll also tell you how long we’ll pay it for.

To register for the Oxygen supplement, you’ll need to be on a Care recipient record.
- Go to the Supplements menus and select Oxygen summary then select Register oxygen.
- Select Confirm to continue.
- Enter the Start date on the Register oxygen screen.
- Select the relevant Oxygen type. If the higher rate Oxygen type option is selected, the screen will display Net Value field. Enter the Net Value amount in dollars and cents. If the start date is from 1 November 2025, the higher rate Oxygen type will no longer be applicable.
- A success message will be displayed to confirm successful registration and that the event is being processed.
- Select Go to oxygen documents to upload the form and supporting documents.
- Select the Document type and then select documents to upload the completed Application for the oxygen and/or enteral feeding supplement form (AC011).
- Upload any other required documents.
