Evidence for Essential Medical Equipment Payment
We may ask you for some evidence to prove you’re eligible for an Essential Medical Equipment Payment (EMEP).
Income
Most types of income count in your income test. We use this and your assets test to assess if you can get a payment, and to calculate your rate of payment.
Scheduled reporting
If you need to report your employment income, it’ll usually be every 14 days, on a date we tell you. We call this scheduled reporting.
Income streams
These are regular payments made from your superannuation fund or purchased using either superannuation money or savings.

Use this form to give us information about your income stream, so we can calculate your payment.
Buying continence aids products
We don’t supply or sell continence aids products.
Essential Medical Equipment Payment - Equipment User Declaration form (SA450)
Use this form to consent to your carer receiving the Essential Medical Equipment Payment instead of you.
Aged Care Carer or close relative assessment form (SA483)
Use this form to give us your details if you’re a carer or close relative of someone moving into an aged care home.
Real estate income
This is lease or rent money you get from a property you own. It counts in your income test.
