Request or update an Individual Healthcare Identifier or request an Individual Healthcare Identifier card form (MS003)
Use this form to register for an Individual Healthcare Identifier (IHI) and request an IHI card.
You’ll already have an IHI if you’re enrolled in Medicare or Department of Veterans’ Affairs (DVA).
Only complete this form if you’re:
- not eligible for Medicare
- not eligible for Department of Veterans’ Affairs pension or benefit
- a visitor to Australia and not covered by a reciprocal healthcare agreement.
You can also use this form to notify the Healthcare Identifiers Service that a person is deceased.
