Eligible care recipients can currently access the End-of-Life Pathway for 12 weeks, with support available for up to 16 weeks where funding remains.
From 8 March 2027, those who remain at home after the initial 12 weeks will automatically get:
- a further 12 weeks of support
- additional funding of about $25,000.
This extends the pathway to 24 weeks.
Providers can continue to claim services under existing arrangements.
