Completing a Centrelink Medical Certificate means we can assess their request faster.
There are different ways you can complete a Centrelink Medical Certificate to update your patient’s Centrelink record.
You can either:
- give us a Centrelink Medical Certificate online using Health Professional Online Services (HPOS)
- fill in the Centrelink Medical Certificate form online or print it and complete it by hand.
Only general practitioners can complete this task. You need a Provider Digital Access (PRODA) account to submit Centrelink forms through HPOS. If you don’t have one, register for a PRODA account. Then follow the steps to set up your HPOS access.
Once you submit the Centrelink Medical Certificate form through HPOS your patient’s Centrelink record will be updated.
Steps to complete a Centrelink Medicare Certificate:
- Select My programs from the HPOS home screen and then select Centrelink forms.
- Select Centrelink forms from the main menu in HPOS.
- Select Medical Certificate option from the page displayed.
- The Medical Certificate instructions screen will be displayed.
- Read the instructions and select Continue.
- Enter the Patient details.
- Select Save and continue.
- Enter the Patient’s primary medical condition(s). All fields are mandatory.
- Select Save and continue.
- Complete the Capacity to work or study details. All fields are mandatory.
- Select Save and continue.
- Next the Certification by Medical Practitioner screen displays the Confidentiality of Information and also displays the Authenticated user provider details.
- Select Save and continue.
- Check the information in the Review and Submit screen. You can use the Update option for each heading of the form to go back and correct any details.
If all the details are correct, select Submit. Your patient’s Centrelink record will be updated.
You can download and complete the Centrelink Medical Certificate (SU415) form.
You can then submit it through HPOS.

You can also give this form and any supporting documents to your patient to lodge with us or they can use their Centrelink online account to upload them.
Authorised medical doctors can complete their own medical certificate form. We’ll accept this as long as it is signed and has the required details. Depending on your patient’s payment type, these can include all of the following:
- what the illness, injury or disability is
- how long it will take for them to recover
- the period they’ll be unable to work, participate or study.
It also needs to tell us if they’re unable to do these for at least 8 hours per week or how many hours they can work, study or participate each week.
