Claiming if you do not have immediate access to the internet or a printer
If you intend to claim from a site without access to the internet you can print a manual Assignment of benefit agreement form prior to the visit. This assignment of benefit agreement allows you to record the claim, patient details and obtain the patient’s signature. These forms are designed for post-service agreement use only.
You may provide a copy to the patient if they request it. When you have access to the internet you can transfer the information into Medicare Bulk Bill Webclaim in Health Professional Online Services (HPOS).

Completing a manual Assignment of benefit agreement
Steps to complete a manual Assignment of benefit agreement:
- Only one patient is allowed per agreement.
- Check date of service is before Medicare card expiry date - indicate with an X in the Expiry date checked box.
- Complete patient details and all other fields that relate to the service provided.
- The Practitioner use box must indicate where required name of hospital, emergency, lost referral, time of service, information about anaesthetists or service duration.
- Patient must sign the Assignment of benefit agreement after the service has taken place and once all the service details are completed. If a patient has already assigned the benefit using another agreement document, including a pre-service agreement, this can be attached to the Assignment of benefit Medicare bulk bill Webclaim form or the Bulk bill voucher - electronically transmitted claims form.
- A copy of the Assignment of benefit agreement must be retained by the health professional for 2 years from the date the agreement is made. This is a legislative requirement.
- When an internet connection is available, access HPOS and Submit a Medicare Bulk Bill Webclaim.
