Use this form to apply to change or recommence PBS-subsidised treatment with an originator biological medicine for adult patients with severe active rheumatoid arthritis.

You can also use this form for patients reclassified from paediatric severe active juvenile idiopathic arthritis or systemic juvenile idiopathic arthritis.

  • adalimumab
  • baricitinib
  • certolizumab
  • etanercept
  • golimumab
  • infliximab
  • tocilizumab
  • tofacitinib
  • upadacitinib.
