Organisation Register change - MyMedicare and other programs form (MO074)

Use this form to notify Services Australia about an organisational change for a practice in the Organisation Register. This includes MyMedicare registered practices.

Organisational change can include:

  • the sale of a practice
  • the merging of practices, including an amalgamation
  • the closure of a practice for a reason other than a sale or a merger
  • a change in Australian Business Number
  • a deceased estate.

You must complete this form within 7 days following an organisational change. The form must be submitted through HPOS. Detailed instructions are provided on the form.
