Australian Immunisation Register immunisation medical contraindication or natural immunity form (IM011)

Use this form if you don’t have access to the Australian Immunisation Register (AIR) or practice management software (PMS). You can use it to report an immunisation medical contraindication or natural immunity.

If you’re an eligible health professional or their Health Professional Online Services (HPOS) delegate, you can quickly record an immunisation medical contraindication or natural immunity:

  • directly in the AIR using HPOS
  • using PMS that supports vaccination reporting.

If you need help linking your HPOS account to AIR, call the AIR and select option 2. If you need help with PRODA, call the PRODA helpdesk.

Contact your software vendor if you’re not sure whether your software can record an immunisation medical contraindication or natural immunity.

If you submit an immunisation medical contraindication or natural immunity using the form upload function in HPOS Messages:

  • you must lodge using the HPOS account of the health professional who signed the form, or submit on their behalf as a nominated HPOS delegate, and show it was sent on their behalf
  • it can take up to 14 days to update the individual’s AIR record
  • we may contact you to confirm the details.

Only eligible health professionals can use this form. You are eligible to use it if you’re a:

  • general practitioner, as defined in the Health Insurance Act 1973
  • paediatrician
  • public health physician
  • infectious diseases physician
  • clinical immunologist.
