Use this form if you’re a registered medical practitioner and either:
- an Australian citizen
- a permanent resident
- a temporary resident
- a New Zealand citizen.
You can also apply for recognition as a specialist or consultant physician using Health Professional Online Services (HPOS).
When applying online, you’ll need a PRODA account to use HPOS.
