Centrelink and aged care nominee forms have changed

You may have noticed changes to our Centrelink and aged care nominee forms.

We recently improved and simplified the Authorising a person or organisation to enquire or act on your behalf (SS313) form. Customers can now use this form to nominate someone to act on their behalf for both aged care costs and Centrelink correspondence.

Department of Veterans’ Affairs (DVA) income support recipients will need to use the Aged Care Request for a nominee for DVA customers (AC019) form to nominate someone to act on their behalf for aged care costs.

This doesn’t affect My Aged Care representatives.

What you need to do

If you have copies of the following forms stored before 11 November 2020, please remove and recycle them as they may not be accepted for processing:

  • Aged Care Appointment of a Nominee (AC019)
  • Authorising a person or organisation to enquire or act on your behalf (SS313)
  • Aged Care Calculation of your cost of care forms (SA456, SA457, SA485).

Please direct customers to the new forms on our website.
